This document can be created by filling in an available template, in paper or digital format. According to the regulations, it is only possible to process the living will under a simple modality through an interview at the Medical Management office of the Regional Hospital. However, the documentation can be prepared in advance to speed up the process and registration.
When processing the living will or prior instructions, it is very important to designate a representative who acts as an interlocutor with the responsible doctor or the healthcare team and who ensures the patient’s instructions contained in the Living will.

- DNI or original passport of grantor and representative and, in addition, a certified photocopy of it.
